Which Questionnaire
Should I Fill Out?
If you have a problem relating to employment, you have these options:
- If
there has been termination for reasons other than disability, or
if there
is more than one issue, the general employment
questionnaire probably is
a good place to start.
- If
you feel you've been placed in a harassing or hostile work environment
because of your sex/gender, race, color, national origin, age or
similar characteristic,
the employment harassment questionnaire should be completed.
- The
disability questionnaire is for someone who has not been hired, not
been accommodated,
or been disciplined or fired because of a disability or serious
medical condition.
The special
education questionnaire is for families who question whether public
schools are providing appropriate special education services to
their children.
The personal injury questionnaire is for anyone who has been injured because
of the negligence or intentional actions of another person or company. A person
can be injured in a car accident, but we also represent people who have been
hurt in nursing homes or other institutional settings. For example, we have
represented children and developmentally disabled persons who have been subjected
to inappropriate
treatment.
We've tried to keep the questionnaires brief (for a lawyer, that's a relative
term), and we realize these forms don't tell the whole story. Again, they are
a way of getting a basic grasp of the problem, and helping us advise if you
have a case worth pursuing.
